Transform Health is a partnership of UK HealthCare providers who have the shared goal of meeting many of the unique medical needs of people who identify as lesbian, gay, bisexual, queer, questioning and/or anywhere on this spectrum, as well as those who consider themselves on the transgender spectrum.

Our clinic is a place for you to feel safe and receive evidence-based care that takes into account all of who you are.

Dr. Keisa Fallin-Bennett, MD, MPH, a provider at UK Family & Community Medicine, and is lead physician for the Transform Health initiative.

Our services

The Family Medicine Transform Health Clinic is located at UK HealthCare-Turfland at 2195 Harrodsburg Road in Lexington. Services include: 

  • Primary care, PrEP (for HIV prevention), hormone therapy for adults and older teens (with consent of parent/guardian), chronic care management, and counseling counseling referrals.
  • Referrals to plastic surgery, urology, gynecology and community mental health providers are available as needed.

General primary care

We are happy to get you established in Transform Health. Call 859-323-6371. Choose option 1 for appointments and ask for Transform Health. If there is a long wait and you prefer to establish primary care with a UK HealthCare provider outside of the Transform Health group, please do not hesitate to do so. We have the utmost confidence in our Family Medicine providers.

Hormone therapy

  • We prefer all new patients come on the designated Transform Health clinic day; we have several providers to do follow-up visits.
  • Note that we do not offer hormone-blocking therapy at this time but will consider hormone therapy for older adolescents with consenting parents/guardians. We are happy to see patients of any age to connect them and their families with information, referrals and resources.

PrEP (pre-exposure prophylaxis/prevention for HIV)

All family medicine providers are capable of starting and maintaining PrEP (or PEP). If you would like to start or maintain PrEP or PEP specifically with the Transform Clinic, please request an appointment with Transform Health. We are happy to have you see our Family Medicine providers outside Transform Health as well.
