Are you a new patient?
If you are a new patient, you will be seen for the immediate problem and a follow-up appointment will be scheduled to establish your regular care with our physicians.
If this is your first visit, please bring your insurance card and forms, medical records (including immunization records), and any medications you are currently taking. When you arrive, go to the registration desk located at Kentucky Clinic on the first or third floor, or in the main hospital lobby.
