What to Bring
- Medical record information (labs, test results, x-rays, etc.) from other facilities
- A list (or the actual bottles) of all medications currently being used, both prescribed and over-the-counter
- Insurance card and any required forms such as a referral from your primary care physician or a completed authorization from your insurance company
- The names and address of all your physicians
- A responsible adult to drive you home (the patient should not drive for 24 hours after receiving sedation for a procedure)

- An overnight bag as there is alway a potential for admission
- A copy advanced directives (living willing, health care surrogate, medical power of attorney, etc) and tell the staff
- Emergency contact information
- A list of past surgeries or procedures, dates and where they were done (open heart, cardiac cath)
*DO NOT BRING valuables, money or jewelry - please leave those items at home or give to a family member for safe keeping.
*Shower with Hibclens (special antibacterial soap) the night before and morning of.
If you would like UK HealthCare to submit a claim on your behalf to your insurance company, it will be necessary for you to present proof of insurance such as a membership card. If your insurance requires you to make a partial payment for certain services (co-pay), this amount will be collected before services are rendered. Payment may be made by check, credit card or cash.
